5-Diamond Patient Safety Program

Program Overview

Renal Network 11 believes it is important for each dialysis facility to incorporate patient safety into their organizational culture. To help promote patient safety values, Renal Network 11 has implemented the 5-Diamond Patient Safety Program. The focus of this program is to help dialysis facilities better implement patient safety principles among both staff and patients.

How Does the Program Work?

The 5-Diamond Patient Safety Program consists of educational modules, which include tools and resources for implementation of each patient safety concept. Each dialysis facility completes one module and submits documentation to Network 11 to receive one "diamond". A maximum of 5 diamonds will be awarded to a facility within the participation period. Upon successful completion of five (5) modules, the facility is recognized as a 5-Diamond Patient Safety Facility.

All facilities are required to complete Patient Safety Principles as their first module. Once this first module is complete, the facility can choose to complete additional modules, including:

How does my facility get started?

  • If you are interested in your facility participating in our new 5-Diamond Patient Safety Program, please complete the application form and fax it to 651-644-9853.
  • The Network office will confirm receipt of your application. You must complete the mandatory Patient Safety Principles module first, and then you may complete additional modules in any order.
  • The current program begins April 1, 2013. Facilities have until March 31, 2014, to complete modules for this program year.Your 5-Diamond Diamond status is good for one year.

Each program year runs on a 12-month cycle. Facilities may extend their status an additional program year by repeating the Patient Safety Principles module, along with two (2) additional modules. When renewing/extending status, facilities may repeat modules previously completed.

Diamonds earned during one (1) program year do not carry over to the next program year. Facilities that do not earn full 5-Diamond status will be recognized as 1-4 Diamond facilities for that program year.


The 5-Diamond Patient Safety Program was developed by ESRD Network 5 and ESRD Network 1
