To establish a collaborative partnership between patients and healthcare professionals, to actively engage patients in their health and to integrate self-management skills to better cope with the challenges of living with and treating chronic illness.

• Complete an in-service training module on Patient Self-Managed Care.
• Complete the Patient Self-Managed Care Perception and Confidence Scale Assessment with patients and discuss their role and responsibilities.
• Encourage patient participation in care plan meetings.
• Complete Patient Self-Managed Care Perception and Confidence Scale Re-Assessment with patients.

• Complete in-service training module 5 A's Model for Self-Management.
• Distribute the Live Your Best Life pamphlet to patients and their families in order to facilitate a patient-centered discussion about the importance of self-managed care.
• Display the 5 A's for Self-Management poster.
• Complete the Self-Management: Healthy Changes Plan worksheet with patients to assist them with short- and long-term healthcare goals.
• Distribute and help patients to complete Your Personal Health Record tool.
• Complete the Patient Self-Managed Care Word Search (Word Search ANSWERS).

• Principles of Motivational Interviewing.
• Quick Tips When Talking with your Doctor.
• New Health Partnerships: Improving Care by Engaging Patients.
• Registration to maintain AAKP electronic "My Health Record".

• 100% of staff in-serviced on patient self-managed care.
• 20% random sample of patients complete the Patient Self-Managed Care Perception and Confidence Scale Assessment and the Re-Assessment.

After you have completed this module, and have documented the required measures, please submit the following documents via mail or fax.
