To create an awareness of patient safety issues associated with slips, trips, and falls in the dialysis unit and what actions can be taken to minimize risks.

• Complete the in-service training module Slips, Trips, and Falls.
• Discuss fall prevention with patients, and have them sign the Patient Fall Prevention Education Form, and have staff document discussions.

• Designate one month as fall prevention month and use posters to educate patients.
• Distribute Falls Prevention at Home brochure.
• Review internal policy for fall risk prevention. (Sample Policy - Fall Risk Prevention)
• Review internal policy for safe escort and movement of patients. (Sample Policy - Safe Escort and Movement of Patients)
• Complete Patient Fall Assessment for each patient.
• Conduct quality improvement project, collecting and analyzing data and implementing intervention (PDCA). (Facility Falls Summary-Monitor for Quality Improvement)
• Post Falls Prevention Checklist in prominent location for staff to use.
• Assure common understanding of terminology among patient care staff. (Are We Talking the Same Language?

• Falls Management Investigation Worksheet.
• Patient Fall Prevention Protocol.
• CDC posters on preventing falls.
• Article: Resources on Reducing Patient Falls.

• 100% of staff in-serviced on slips, trips, and falls.
• 85% of patients sign Patient Falls Prevention Education form and have documentation in medical record.

After you have completed this module, and have documented the required measures, please submit the following documents via mail or fax.
