To increase staff and patient knowledge of transplantation as a preferred modality for treatment of ESRD and to implement the position of facility-level Transplant Liaison.

• Complete the in-service training module Transplantation - Shared Responsibility. (with speaker notes)
• 100% of staff complete pre- and post-tests.
• Use tracking mechanism to ensure that all new patients receive transplantation education.
• Use tracking mechanism to ensure monthly lab samples are submitted for all patients on the transplant waitlist.
• Assign and submit the name of the healthcare worker (HCW) in the role of dialysis facility transplant liaison.

• Distribute the Transplantation Glossary to facility staff and all eligible patients.
• Review your facility's policy and procedures for patient transplantation education.
• View an educational video on transplant.
• Complete the Transplant Word Search (answers).

• Transplant Question and Answer.
• Transplant Question and Answer Resources
• Transplant Option Tool.

• 100% of staff in-serviced on Transplantation.
• 100% of staff complete pre- and post-Tests.
• Identify facility-level Transplant Liason.

After you have completed this module, and have documented the required measures, please submit the following documents via mail or fax.
