Provider Directory
The provider directory is available in two formats: (1) An interactive search utility. The data from this utility is updated monthly with changes reported by facilities. You can use the utility to search specific areas of interest by city or by state. (2) A printable PDF of the complete directory. The data published in this document reflects a "snapshot in time" from updates made in the Spring of each year. The document also includes a Resources section and some introductory information regarding ESRD Networks.
Search the Network 11 provider directory by city and state. To view all facilities in a state, leave the city field blank.
Search Tips
- To find a facility near you, use Mapquest (link opens in new browser) to find cities near you, then search for them here.
- Don't use punctuation. For instance, St. Louis Park should be typed St Louis Park.
- Use full city names. For instance, if St Paul doesn't return any results, try Saint Paul, and vise-versa.
- Be aware of spaces in city names. For instance, LaCrosse, WI could be listed as LaCrosse or La Crosse. Try all variations.
- You can use the % sign as a wild-card. For example, you're looking for a facility in La Crosse, WI, but are unsure of the spelling. Typing La% in the city box, and selecting WI in the state box will result facilities in all cities in WI beginning with La.
